Ideal Dimple Placement on the Cheeks

dimple locations

Creating natural-looking cheek dimples takes both artistic sensibility and a deep understanding of facial anatomy. Whether you’re considering dimpleplasty for a playful, youthful look or a subtle enhancement, here’s how surgeons typically determine the ideal placement:

Anatomy Matters for Dimple Locations

Dimples are usually formed where there is a natural indentation caused by a defect or tethering of the buccinator muscle. This typically occurs in the malar (cheek) region, a few centimeters lateral to the corner of the mouth.

What is The “Dimple Line”

Many surgeons reference two intersecting lines to identify potential placement: a vertical line dropped from the outer corner of the eye (lateral canthus) and a horizontal line extending from the corner of the mouth.
Others take a slightly more medial approach, placing the dimple where this diagonal line intersects with a vertical line dropped from the outer edge of the iris when the patient looks straight ahead.

Facial Movement and Expression

Since natural dimples often appear with smiling, facial movement is part of the assessment. Some surgeons ask patients to smile or gently suck in their cheeks to identify the deepest point.
Another common technique is to place the dimple slightly higher—along the line between the corner of the mouth and the peak of the Cupid’s bow—for a softer, more elevated appearance.

Symmetry and Personal Preference

Some patients prefer symmetrical dimples, while others desire a single dimple on one side. During consultation, your surgeon will take your facial symmetry and personal aesthetic goals into account.

Customization Is Key

There’s no universal placement that fits everyone. Skin thickness, cheek volume, muscle tone, and facial proportions all influence the final location. A skilled surgeon will assess your anatomy and tailor the placement to ensure the most flattering and natural result—while incorporating your preferences.
